Your First Visit

At Dr. Catherine Holt’s office, you will be warmly welcomed from the moment you step through the door. After completing your New Patient paperwork, you will have a personal interview with Dr. Holt prior to your exam.  She will then discuss your personal needs with you and any goals you may have for the future, treating you like family and helping you stay comfortable throughout your entire visit.

Instead of having to wait until you reach our office for your first appointment, patients can now download, print, and complete the new patient forms they will need in the comfort of their own home.
